Below is a picture of the table of contents of a pest control log that is kept at an audited account.
The table of contents has the sections on the left and the actual content on the right.
All the content on the right is required i.e. pest sighting logs, monitoring logs, service reports, trend reports, licenses, insurance, W9 etc…
It should be cleared with the account that service reports and trend reports will be accessed online.

For audited accounts we need to carry 3 different insurance policies:
- Commercial General Liability
- Automobile Liability
- Workers Compensation and Employers Liability
Because there are 3 different insurance policies, 3 different must be checked to make sure none of them are expired.
Here’s what the insurance document looks like:

Pest control licenses must also be checked to make sure they are not expired.
Typically we will list 3 different licenses:
- Business license
- Technician license
- Manager license
The business license says “PESTICIDE APPLICATOR BUSINESSS” on the front. Like all New Jersey pest control licenses it expires in October of every year. It looks like this:

The technician and manager licenses look the same. It needs to “COMMERCIAL PESTICIDE APPLICATOR” on the front and also show the applicator is licensed for food processing i.e. 7D. Like all New Jersey pest control licenses they expire in October every year.
